Hormone Wellness Care

Understanding Your Hormones

Hormones influence much more than your menstrual cycle. Estradiol, progesterone, and testosterone interact with tissues throughout the body and can influence reproductive health, bones, the brain, sleep, sexual function, and more.

As hormone patterns change during perimenopause and menopause, symptoms can appear throughout the body; not just hot flashes.

Estradiol

Estradiol is the primary estrogen during the reproductive years, with receptors throughout the body.

Estradiol plays an important role in:

Brain & temperature regulation — Influences thermoregulation, mood, cognition, and sleep.

Bone health — Helps maintain bone density by regulating bone remodeling. Declining estrogen after menopause contributes to accelerated bone loss.

Vaginal & urinary health — Supports vaginal tissue thickness, elasticity, lubrication, blood flow, and the urinary tract.

Cardiovascular system — Estrogen has effects on blood vessels, lipid metabolism, and other aspects of cardiovascular physiology.

Skin & connective tissue — Contributes to collagen production, skin thickness, hydration, and elasticity.

Declining estrogen can contribute to hot flashes, night sweats, vaginal dryness, painful intercourse, urinary symptoms, sleep disruption, and bone loss.

Progesterone

Progesterone rises after ovulation during the reproductive years and plays an essential role in the menstrual cycle and pregnancy.

Progesterone is important for:

Uterine protection — Progesterone counteracts estrogen’s effects on the endometrium. When systemic estrogen is prescribed to someone with a uterus, adequate progestogen is generally needed to protect against endometrial hyperplasia and cancer.

Menstrual regulation — The normal rise and fall of progesterone helps regulate the menstrual cycle.

Pregnancy — Progesterone prepares and maintains the uterine lining to support pregnancy.

Brain & sleep — Progesterone metabolites interact with GABA-A receptors in the brain, which may contribute to sedative effects and changes in sleep.

During perimenopause, ovulation may become inconsistent, so progesterone patterns can change even while estrogen continues to fluctuate.

Testosterone

Women naturally produce testosterone throughout life. It is produced by the ovaries and adrenal glands and is also formed through the conversion of other hormones. Testosterone is an important part of normal female physiology and acts on androgen receptors throughout the body.

Testosterone contributes to:

Sexual function — Androgens play a role in sexual desire, arousal, and sexual response.

Muscle & bone — Testosterone contributes to the maintenance of muscle and bone physiology.

Brain & overall well-being — Androgens have effects within the brain and other tissues involved in normal female physiology. Research continues to explore the relationship between testosterone and areas such as cognition, mood, energy, and overall well-being.

Overall androgen function — Testosterone acts on androgen receptors throughout the body and is part of normal female physiology and is not simply a “male hormone.”

Testosterone Therapy for Women

Testosterone therapy has its strongest evidence for the treatment of hypoactive sexual desire disorder (HSDD) in appropriately evaluated postmenopausal women.

For some women in the later reproductive years or perimenopause, testosterone therapy may also be considered after a comprehensive evaluation, although research in premenopausal women is more limited. For individuals who could become pregnant, pregnancy prevention and reproductive plans are important considerations because testosterone therapy should not be used during pregnancy.

Treatment is individualized based on symptoms, medical history, reproductive status, treatment goals, and appropriate monitoring.

Hormone Therapy Is Personalized

Hormone therapy isn’t about achieving a universal “perfect” hormone level.

Your treatment plan considers your symptoms, stage of the menopause transition, medical history, risk factors, treatment goals, and clinical evaluation. Laboratory testing may provide additional information when clinically appropriate.

Treatment may include estradiol, progesterone, testosterone when indicated, vaginal hormone therapy, nonhormonal medications, supplements, or other therapies depending on your individual needs.
